Email Notifications – Sign up form layout 6

In this example, we are showing layout 6.
We are using a Group Filter and have the press list as the only list in this group filter.
We have ca=”1″ to check all lists by default.
We have ha=”1″ for hide all lists so the user only enters their email and clicks subscribe.
